Our mission




uniting to fight heart disease

Having witnessed first hand how much suffering preventable heart disease causes in our community, we knew something needed to be done. With the help of our partners and community activists, the Center for Well-Being and United Way are working together to fight heart disease in Sonoma County. Our aim is to educate the community, build local support systems, and encourage action that will end preventable heart disease. We believe that with greater information and support, our communities will grow healthier and stronger. It’s time to stop preventable heart disease - it’s up to us


Our mission is to improve lives by mobilizing the caring power of our community through leadership, community engagement and effective investment.

Hearts of Sonoma County is a cardiovascular disease risk reduction initiative of the Committee for Healthcare Improvement, a committee of Health Action. CHI membership consists of healthcare providers and community stakeholders united to achieve person-centered, high value, well-coordinated care for all Sonoma County residents.

The vision for Hearts of Sonoma County is to significantly reduce heart attacks and strokes as the major killer of people in Sonoma County through community engagement and active collaboration across health providers to inspire health and health equity, with the goal of early detection and better management of high blood pressure and other risk factors.
